Terms & Conditions — Lactation Consulting Services
These Terms & Conditions ("Terms") govern your use of lactation consulting services (“Services”) provided by [Consultant Name], doing business as [Business Name] ("Consultant," "we," "our"). By scheduling, attending, or paying for Services you agree to these Terms. If you do not agree, do not use the Services.
Services Provided
We provide lactation consultation services, including assessment, education, feeding support, pump/technique guidance, breastfeeding plans, and written resources. Services may be delivered in-person, via telehealth, or by phone/text, as agreed.
All consultations are advisory and educational. We do not diagnose medical conditions or replace medical care. If a medical condition is suspected, we will recommend referral to an appropriate healthcare professional.
Eligibility and Informed Consent
By consenting to Services you attest that you are authorized to make health decisions for the client and infant.
You agree to provide accurate, complete, and current health and contact information. You understand that incomplete or inaccurate information may limit the effectiveness of the Services.
Appointments, Scheduling, and Cancellations
Appointments must be scheduled in advance. Appointment length and format will be confirmed at booking.
Cancellation and rescheduling policies: Cancellations made at least 24 hours before the scheduled appointment will receive a full refund or credit. Cancellations less than 24 hours before the appointment may be subject to a cancellation fee equal to [percentage or flat fee] of the session rate or forfeiture of any prepaid fee.
No-shows: Failure to attend a scheduled appointment without notice may be charged the full session fee.
Late arrivals: If you arrive late, the session may be shortened to end at the scheduled time; you may still be charged the full fee.
Fees, Payment, and Billing
Fees for Services will be provided at booking and are payable as specified (e.g., at time of service or in advance). We accept payment by [accepted payment methods].
Packages, group sessions, or subscription plans are non-transferable unless otherwise agreed in writing.
Refunds: Refunds are provided only as specified in these Terms or at our discretion. Prepaid packages may have expiration dates; unused sessions after expiration may be forfeited.
Insurance and reimbursement: We may provide receipts or superbills for insurance reimbursement where applicable, but we do not guarantee coverage. You are responsible for verifying benefits and submitting claims.
Telehealth and Remote Services
Telehealth consultations require a stable internet connection, a private location, and appropriate devices. We are not responsible for technical failures that interrupt or prevent a session.
By using telehealth, you consent to electronic communication and release us from liability arising from technical issues, unauthorized access, or data breaches beyond our reasonable control.
Privacy and Confidentiality
We maintain confidentiality of client information in accordance with applicable laws and professional standards. We will not disclose your protected health information except as required by law, to provide Services, or with your written consent.
For telehealth and electronic communications, some privacy risks exist. We will use reasonable safeguards but cannot guarantee complete security.
Professional Boundaries and Scope of Practice
Our role is limited to lactation consultation. We will not provide medical diagnosis, prescriptions, or emergency care. We will refer to medical providers when indicated.
We will maintain professional boundaries. Personal relationships between consultant and client beyond the professional scope are discouraged and may end the professional relationship.
Risks and Limitations
You acknowledge that breastfeeding and infant feeding involve risks (e.g., nipple damage, infection, infant weight issues, choking, aspiration). We will offer evidence-informed guidance but cannot guarantee outcomes.
You accept responsibility for implementing recommendations safely and for seeking medical care when necessary.
Consent to Photographs and Video (Optional)
With prior written consent, we may take photographs or video for clinical documentation or educational purposes. Identifiable images will not be used publicly without explicit written permission.
Record Keeping and Retention
We maintain clinical records for a period required by law or professional guidelines. You may request copies of your records; reasonable fees for copying may apply.
Client Responsibilities
You agree to attend scheduled sessions prepared, to follow agreed infection-control practices during in-person visits, and to communicate changes in health status promptly.
You agree to follow safety advice for infant positioning, latching, and feeding. If you notice signs of infant distress, you will stop feeding and seek immediate medical help.

Introduction These Terms and Conditions (“Terms”) govern the provision of lactation consultation services by [Consultant Name/Practice Name] (“Consultant,” “we,” “our,” or “us”) to the client (“Client,” “you,” “your”). By scheduling, attending, or receiving services, you agree to be bound by these Terms. If you do not agree, do not schedule or attend services.
Scope of Services
Services provided may include in-person consultations, virtual consultations (telehealth), phone consultations, assessment of breastfeeding/lactation issues, education on feeding techniques, personalized care plans, follow-up recommendations, and referrals to other healthcare providers as appropriate.
The Consultant does not provide medical diagnosis, medical treatment, or emergency services. Consultants may provide lactation-related assessments and recommendations within their scope of practice and qualifications.
Qualifications and Licensing
The Consultant will disclose professional credentials (e.g., International Board Certified Lactation Consultant [IBCLC], Certified Lactation Counselor [CLC], registered nurse) upon request.
Services are provided in accordance with applicable professional standards, laws, and regulations.
Appointments, Scheduling, and Missed Visits
Appointments must be scheduled in advance. Please arrive on time for in-person visits and be available at the scheduled time for virtual or phone visits.
Cancellations: If you need to cancel or reschedule, notify us at least [time period, e.g., 24 or 48] hours before the appointment. Cancellations made within the specified window will not be charged a cancellation fee.
Missed appointments or late cancellations (within the specified window) may incur a cancellation fee of [amount or percentage] or forfeiture of the appointment fee. Repeated no-shows may result in refusal of future scheduling.
Fees, Payment, and Insurance
Fees: Consultations are charged at the rates disclosed at the time of booking. Additional charges may apply for extended sessions, travel for home visits, written reports, or document preparation.
Payment: Payment is due at the time of service unless other arrangements are made in advance. Acceptable forms of payment will be provided at booking.
Insurance: Some insurance plans may reimburse lactation consultation services. It is the Client’s responsibility to verify coverage and submit claims for reimbursement where applicable. We may provide receipts or superbills on request.
Returned payments, failed card transactions, or overdue balances may incur additional fees and may affect future scheduling.
Telehealth/Virtual Visits
Telehealth services are subject to the same Terms. Client must have access to a secure internet connection and compatible device. The Consultant is not responsible for technical failures that prevent or interrupt a virtual visit.
Confidentiality measures for telehealth will be taken, but the Consultant cannot guarantee the security of third-party platforms. By using telehealth, you consent to its limitations.
Client Responsibilities
Provide accurate medical history, relevant health and feeding information, and any requested documentation (e.g., infant weight records, feeding logs, medical forms).
Follow pre-visit instructions (e.g., bringing supplies, preparing infant) and follow post-visit care plans and recommendations as advised.
Seek immediate medical care or emergency services when signs of illness, severe pain, bleeding, or other urgent symptoms occur. The Consultant may recommend consultation with or referral to a medical provider when appropriate.
Informed Consent and Shared Decision-Making
Recommendations are based on clinical judgment, best available evidence, and client goals. Clients remain responsible for decisions about care and feeding practices.
The Consultant will obtain informed consent for recommended interventions and will explain risks, benefits, and alternatives.
Referrals and Collaboration with Other Providers
The Consultant may refer to physicians, pediatricians, obstetricians, mental health professionals, lactation specialists, or other healthcare providers as needed.
With client consent, the Consultant may communicate with other providers involved in care to coordinate services.
Confidentiality and Privacy
Client information will be treated as confidential and maintained in accordance with applicable privacy laws. Personal health information may be used for treatment, billing, and practice operations.
The Consultant may use de-identified information for education, quality assurance, or research without obtaining additional consent.
Requests for release of records require written authorization except as required or permitted by law.
Records and Documentation
Clinical records are maintained by the Consultant in accordance with professional standards and legal requirements. Clients may request copies of their records; fees for copying and mailing may apply.
Limits of Practice and Liability
The Consultant provides lactation support within their scope of practice. The Consultant is not responsible for adverse outcomes arising from: (a) failure to follow recommendations; (b) delays in seeking medical care; (c)
